You must apply in person only if you qualify for an appointment If:
You are applying for your first U.S. passport; or
You are under age 16; or
Your previous U.S. passport was issued when you were under age 16; or
Your previous U.S. passport was lost, stolen, or damaged; or
Your previous U.S. passport was issued more than 15 years ago; or
Your name has changed since your U.S. passport was issued and you are unable to legally document your name change.
Apply in person at:
Shakopee Government Center (passport desk) across from Customer Service. Before making an appointment, you need to visit the Department Of State's website and print out the DS 11. We encourage the use of the online "form filler" - please follow the directions on their website to complete the application. You must bring a completed, unsigned application with you for your appointment. Expedited passports are not available at this time.
You must also bring in a checkbook or money order(s) for the US Department of State fees (one per application) NO CASH IS ALLOWED.
